I'm working on a second edition of Plain English for Cops.  I'd like to get some representative policies on your department's format for the narrative portion of the report.  For example, one department uses this format for the narrative:

Source of Activity

Weather Conditions

Observations on Scene

Investigative Action Taken

Follow-up

Another department uses this format:

Arrest

Source

Circumstance of Incident

Observations by Officers

Taken (evidence, property, impound)Information

Another uses this format:

• Initial Dispatch Information

• Venue

• Date and Time

• Victim

• Interview with Victim

• Suspect

• Interview with Suspect

• Witnesses

• Interview with Witnesses

• Arrests/Warrant Authorization

• Follow-up investigation

• Status

What narrative structure does your department use?  If I use your submission, I'll acknowledge you in the second edition.

Thanks.
